Provider/Community Grants Program

Thank you for your interest in the Home State Health Grant Program. Grants of up to $15,000 are available to participating Home State Health providers and community organizations focused on promoting the health and well-being of the communities we serve.

Beginning in 2022, we will be revamping the grant selection process to better align with the U.S. Department of Health and Human Services' Health People 2030. This will ensure that the grants Home State Health provides directly combat the Social Determinants of Health (SDoH) our members and all Missourians are most affected by.

Deadlines and applicable SDoH domains for submissions are as follows:

  • Q1 & Q2 - June 30, 2022
    •    Neighborhood and Built Environment (i.e. Access to Quality and Affordable Housing)
    • Education Access and Quality
  • Q3 & Q4 - December 29, 2022
    • Health Care Access and Quality
    • Social and Community Context (i.e. Health Literacy and/or Nutrition and Healthy Eating)

Applications must be completed in full and submitted online to be considered. All applications are reviewed by Home State Health and will be awarded on a bi-annual basis. Applicants will be notified of the committee's decision by end of the month following quarter end.

Please note that submitting an online application does not guarantee funding. Please contact alexander.c.vandeloo@homestatehealth.com with any additional questions.
