Show Me Healthy Kids (SMHK) is Missouri’s dedicated Medicaid managed care health plan for children and youth in foster care, former foster care, adoption subsidy, and select juvenile justice programs.
Administered by Home State Health, SMHK provides comprehensive, coordinated medical, behavioral health, and support services. The plan offers specialized, person-centered services designed to address the unique physical, emotional, and social needs of itsmembers — supporting their health, stability, and overall well-being.
Individuals who are eligible for coverage through SMHK are automatically enrolled in the plan and will maintain their coverage for as long as they remain eligible.
Covered eligibility groups include:
- Children in the care and custody of the Missouri
Children's Division - Youth in the custody of the Missouri Division of Youth
Services - Children receiving adoption or legal guardianship
subsidy - Former foster care youth under the age of 26, who were
in foster care on their 18th birthday and covered by MO HealthNet (Missouri Medicaid) or Medicaid from another state, and who meet other eligibility criteria
