Healthy Activities Eligible for Rewards
Take Healthy Steps to Earn My Health Pays™ Rewards.

| Cervical Cancer Screening | (Age 21-64) One screening in a 3-year period |
| Annual Breast Cancer Screening | (Age 40-74) One per calendar year |
| Participation in 2 Tobacco-Free Counseling Sessions | Must complete 2 sessions a calendar year. (Age 18 and up) |
| Lead Screening Test | Must be completed before child’s second birthday. Once per lifetime |
| Well Care Visit With Your Assigned Primary Care Doctor | Within 90 days of enrollment |
| Completion of At Least 3 Health Coaching Calls | 3 per calendar year. You must be assigned to case management for asthma, depression and/or diabetes to be eligible for this reward |

| For Every 3rd Prenatal Doctor visit | To be eligible for this reward, you must notify us you are pregnant prior to having your baby by calling us or submitting a completed Notification of Pregnancy (NOP) form |
| Postpartum Doctor Visit | 4-6 weeks after delivery. To be eligible for this reward, you must notify us you are pregnant prior to having your baby by calling us or submitting a completed Notification of Pregnancy (NOP) form |
| Telehealth Visit | One per calendar year. Home State Health offers telehealth services through our Video Chat with a Doctor program. Your doctor may also offer telehealth services. |
| COVID-19 Vaccine | Eligible members who receive their full* COVID-19 vaccination *Member must receive at least two doses of the Pfizer or Moderna vaccine, or one dose of the Johnson and Johnson vaccine |

Your My Health Pays reward dollars are added to your rewards card after we process the claim for each activity you complete. If you are earning your first reward, your My Health Pays™ Visa® Prepaid Card will be mailed to you.
This My Health Pays Rewards Visa Prepaid card is issued by The Bancorp Bank pursuant to a license from Visa U.S.A. Inc. The Bancorp Bank; Member FDIC. Card cannot be used everywhere Visa debit cards are accepted.

Once you complete a qualifying healthy activity, we are notified, and your card will be mailed to you. Remember to keep your card, as future reward dollars will be added to it for each qualifying healthy activity you complete. If you have questions, please contact us at 1-855-694-HOME (4663).
Once you receive your My Health Pays™ rewards card in the mail, please follow the instructions on the activation sticker. You will need to have your card with you to complete activation.
To check your balance, login to your Secure Portal account.
Funds expire 90 days after termination of coverage or 365 days after date reward was earned, whichever comes first.
