Care and Disease Management

We want you to have the best quality of life you can have.

Home State has several programs to improve the health of our members. We know this means more than just helping you to see a Primary Care Provider (PCP). It also means helping you find your way through the healthcare system to you get the treatments and the social services you need. It also means helping you understand and manage your health conditions. We do this through education and personal help from Home State staff. The goal of this program is to add to the quality of your care and help you improve your health. Special health programs include care and disease management. Special Healthcare Needs

If you have a special healthcare need, call Home State at toll-free at 1-855-694-HOME (4663). Home State will work with you to make sure you get the care you need. If you have a chronic illness and are seeing a specialist for your medical care, you may ask Home State for a specialist to be your PCP.

Disease Management Services

Home State works with Envolve to give disease management services to our members. Envolve Health Coaches know a lot about conditions like:

  • Asthma
  • Depression
  • Diabetes

They will talk to you about:

  • How to take medications
  • What screening test to get
  • When to call your PCP

The Health Coach will listen to your concerns and help you take steps towards better health. The goal of disease management is to help you understand and take control of your condition. Better control means better health.

Care Management Services

Our care managers are nurses that can help you understand your health problems. Our nurse works closely with you and your healthcare providers to help you get the care you need. The nurse can set up home health or other needed services. Home State will give care management services for members who are

  • pregnant or
  • children ages 6 years or younger with elevated blood lead levels.

Within thirty (30) days of enrollment, Home State will do a care management assessment for new members with the following conditions:

  • cancer,
  • cardiac disease,
  • chronic pain,
  • hepatitis C,
  • HIV/AIDS,
  • children with special healthcare needs including autism spectrum disorder.
  • sickle cell anemia,
  • anxiety disorders and
  • pervasive developmental disorder.
